District Nurses

District nurses are employed by South West London ICB. Their duties include visiting patients at home when they are unable to come to the surgery.

Each patient’s needs are assessed and the care/treatment is planned accordingly. The community nurses provide a domiciliary phlebotomy service in appropriate instances.

Social Prescriber Clinic

This clinic runs weekly, and is provided by one Thornton PCN. The social prescriber will assist you with:

  • Benefits
  • Weight Management and signposting weight management resources
  • Mental Health Services
  • Housing and other services available in the community.
